Aqui está uma solução que parece funcionar:

Analisando isso com EXPLAIN mostra que ele emprega índices muito bem:

Em geral, você quer evitar planos de otimização que forçam Using filesort ou Using temporary porque estes são assassinos de desempenho. Uma consulta usando GROUP BY é quase certo que causará esse tipo de otimização, pelo menos no MySQL.
